Broad Function:

Responsible for accurately collecting and entering patient medication histories into the hospital’s information system for utilization by the Physician responsible for admitting, discharging, or releasing the patient from the hospital. Is responsible for relaying any issues that cannot be addressed by the assistant to the pharmacist.

 

Major Responsibilities:

1. Ensures that they cultivate a work climate supportive of the Hospital’s mission, vision and values.

2. Commits to a customer service-oriented culture within his/her area which includes service orientation to work processes, staff customer service and actively participates and supports development of key customer service outcomes.

3. Uses equipment and supplies safely and properly, without waste.

4. Communicates in a courteous and tactful manner with co-workers and visitors.

5. Arrives for work punctually/punches in and out daily.

6. Uses meal and coffee breaks appropriately.

7. Uses EBT and PBT appropriately.

8. Participates in appropriate educational programs. Continues to increase knowledge of medications to perform more effectively.

9. Follows dress code policies; maintains a neat and clean appearance.

10. Assists in identifying needs of the department, e.g. education, supplies, repairs, etc.

11. Assists co-workers willingly and shares knowledge for the benefit of co-workers.

12. Makes adjustments in workload to cover extra work. Is responsible for ensuring that all assistant tasks are completed by the end of the day or communicates items left to next shift.

13. Identifies cost-saving ideas that will contribute to the well-being of the hospital.

14. Maintains work area in a clean an orderly condition.

15. Continually demonstrates attention to detail and accuracy.

16. Demonstrates the necessary skills and knowledge to complete day to day activities required according to the specific assignment.

17. Signs off on all work that is completed.

18. Maintains patient confidentiality at all times

 

Medication Reconciliation

1. Collects a list of the patient’s current home medications in anticipation of admission.

2. Is responsible for gathering all information that is needed to completely and accurately enter into the hospital’s computer system.

3. Is responsible for obtaining additional medication information from outside sources when necessary or practical (including physician’s office, mail-order pharmacy, retail pharmacy, extended care facility, or another acute care facility).

4. Effectively communicates with other healthcare professionals to maintain adequate workflow and identify patient specific needs (i.e. language barriers).

5. Accurately enters the home medication list into the hospital’s information system for utilization by the Provider responsible for admitting or discharging the patient from the hospital.

6. Completes the computer entry of the home medication list in a timely manner so it can be used as the patient’s admitting orders.

7. Directs any unresolved issues to the pharmacist.

8. Assist the main pharmacy with duties to provide medications for patients in a timely manner.

9. Performs other duties as assigned.

 

Qualifications:

  • H.S. graduate with certified Pharmacy Technician License or one (1) year pharmacy experience.
  • Must be able to interpret and perform mathematical computations.
  • Obtain and maintain certified pharmacy technician certificate from a nationally recognized program within one (1) year from date of hire; if not already certified.
  • Must obtain and maintain a current NYS Pharmacy Technician License within one (1) year from date of hire; if not already certified.

 

Job Type: Part-time

 

Pay: $18.24 – $20.93 per hour

 

Expected hours: 30 per week
